Life care planning is the process in which a comprehensive medical and rehabilitation plan is generated for individuals who have experienced severe injury or have chronic health care needs.
Vocational rehabilitation is the systematic process by which people with disabilities access the resources and the knowledge necessary to be gainfully employed.
Medical case management is the coordination of appropriate health and social services so that an injured or disabled person can reach optimum physical and mental functioning.
